How on earth does the government expect to incentivise elderly people to move out of homes they may have lived in for more than 50 years?

If the government seriously wants older people to give up their family homes they need to provide far better care homes. Most of the elderly care homes are very poor with uncaring staff who just go through the motions and do not really care about the residents.
There are many older people who have no family to help them or to leave their homes to who struggle with every day task but are afraid of going into a care home because of the bad reputation so many of them have.

Personally I think it is scandalous that the government is even talking of “incentivising” elderly people to sell their homes. If they want to move out and downsize then they are free to do so without encouragement from the authorities. You can bet your bottom dollar, if they were to move into a care home then the authorities would claw back as much money as possible from their property sale.
