Hello, any help with mortgage name change

Hello, I am new to the forum and have found myself here due to the following situation.

So this is an introduction as well as a question.

I own a property with my partner. It is a house in London. The property is in her name but all payments from the first until now have come from my bank account.
Unfortunately we will be parting ways and would like to know if it is possible to transfer the property (mortgage) to my name to enable her to move on and myself to retain the property?

Any advice would be very welcome.
